Form Items

What are Form Items?

Form items are the components that make up your form.

Each form item counts as one item towards your form's Items per Form limit, unless otherwise noted.

Blank Space

A Blank Space item can be used to separate other items for organization.

Calculation

A Calculation item calculates the value of an equation. The value will be recalculated after a page submission, or when form users click the "Calculate" button next to it. For complete information, see the Calculations guide.

Calendly

A Calendly item integrates a Calendly booking flow into your form. Form users can schedule without leaving your form.

Checkbox

A Checkbox item provides multiple choices and allows form users to select one or more.

Checkbox Email Routing

A Checkbox Email Routing item can be used to send Notifications to the addresses associated with form users' responses. After you add this item, create a Notification that uses it as a "To" or "From" address.

Checkbox Matrix

A Checkbox Matrix item allows form users to choose one or more of multiple choices with multiple questions pertaining to the answers in a matrix display. Each matrix row counts as one item towards your form's limit. Matrix items are limited to 50 rows each.

Checkbox Multi Scale

A Checkbox Multi Scale item allows form users to choose one or more of many answer groupings with multiple questions pertaining to the possible answers. Each multi scale row counts as one item towards your form's limit. Multi Scale items are limited to 50 rows and 50 answer choices each.

Checkbox Pricing

A Checkbox Pricing item is a Checkbox item that can be assigned order form prices.

Coupon Code

A Coupon Code item provides coupon code functionality for order forms. A Coupon Code can give either a fixed or percent discount, but not a mix of both.

Credit Card

A Credit Card item provides a validated secure credit card field. Use this item to collect credit card numbers when you're not using payment integration.

Custom Code

A Custom Code item provides space for advanced users to insert HTML or JavaScript code.

Date

A Date item provides a validated date field with a popup calendar. When a form user selects a date on the popup calendar, the text field will automatically be filled in to match.

Description Text

A Description Text item provides space to display extra text or instructions, including links and images.

Dropdown

A Dropdown item provides multiple choices and allows form users to select only one.

Dropdown Email Routing

A Dropdown Email Routing item can be used to send Notifications to the addresses associated with form users' responses. After you add this item, create a Notification that uses it as a "To" or "From" address.

Dropdown Matrix

A Dropdown Matrix item allows form users to choose one of multiple choices with multiple questions pertaining to the answers in a matrix display. Each matrix row counts as one item towards your form's limit. Matrix items are limited to 50 rows each.

Dropdown Multi Scale

A Dropdown Multi Scale item allows form users to choose one of many answer groupings with multiple questions pertaining to the possible answers. Each multi scale row counts as one item towards your form's limit. Multi Scale items are limited to 50 rows and 50 answer choices each.

Dropdown Percent

A Dropdown Percent item provides a way to apply a percentage to an order total, such as for a tax or shipping charge. Other Order Form items can be included in or excluded from percentage calculations.

Dropdown Pricing

A Dropdown Pricing item is a Dropdown item that can be assigned order form prices.

Email Address

An Email Address item provides a validated email address field. After you add this item, you can create email Notifications that use your form users' responses as a "To" or "From" address.

File Upload

A File Upload item provides an upload field to collect one or more files.

Heading

A Heading item provides a divider with text to separate different sections of a form.

Hidden Field

A Hidden Field is an invisible text field. This item can be used to hold data that should not be seen by form users, such as a customer number or link source. You can use the Pre-Populate feature to pass data into a Hidden Field.

Image

An Image item provides a way to display an image such as a logo, product sample, or decoration. Professional level accounts can upload custom images.

Image List

An Image List item provides form users with multiple image choices and allows one or more to be selected.

Image List Pricing

An Image List Pricing item is an Image List item that can be assigned order form prices.

Image Quantity List

An Image Quantity List item provides a way to add images for multiple rows of quantities. Each quantity row counts as one item towards your form's limit.

Long Answer

A Long Answer item provides space for a multiple line reply of nearly unlimited length.

Long Answer Matrix

A Long Answer Matrix item provides a set of text areas in the form of a grid. Each text area counts as one item towards your form's limit.

Number

A Number item provides a validated number field. Values for this item are handled as number values rather than text values. For example, when using a Short Answer item in Rules, checking if "100.00" is equal to "100" is false because the text values are not equal. But when using a Number item in Rules, checking if "100.00" is equal to "100" is true because the number values are equal. When using decimal places, numbers are rounded and formatted to the specified number of places.

Number Scale

A Number Scale item allows form users to choose a number between 0 - 10.

Page Break

A Page Break item splits a form into multiple pages. Do not place a Page Break as the first or last item on a form or place two Page Breaks directly next to each other.

Phone Number

A Phone Number item provides a validated phone number field.

Quantity List

A Quantity List item provides multiple rows of quantities. Each quantity row counts as one item towards your form's limit.

Radio Button

A Radio Button item provides multiple choices and allows form users to select only one.

Radio Button Email Routing

A Radio Button Multi Scale item allows form users to choose one of many answer groupings with multiple questions pertaining to the possible answers. Each multi scale row counts as one item towards your form's limit. Multi Scale items are limited to 50 rows and 50 answer choices each.

Radio Button Percent

A Radio Button Percent item provides a way to apply a percentage to an order total, such as for a tax or shipping charge. Other Order Form items can be included in or excluded from percentage calculations.

Radio Button Pricing

A Radio Button Pricing item is a Radio Button item that can be assigned order form prices.

Ranking

A Ranking item provides text fields to rank in order from 1 to X. Each Ranking row counts as one item towards your form's limit. Ranking items are limited to 50 rows each.

Rating

A Rating item provides text fields with weights or points that must add up to a set amount. Each Rating row counts as one item towards your form's limit. Rating items are limited to 50 rows each.

Running Total

A Running Total item allows form users to calculate their current Order Form order total. The current total will be recalculated after a page submission, or when form users click the button next to it. This item is optional; the final itemized order total is always displayed on the Order Summary page, after the last page of a form.

Save & Return

A Save & Return item enables the Save & Return feature and adds a login link to your form. Adding this item is a shortcut for using the settings on your form's "Form Settings -> Save & Return" page.

Short Answer

A Short Answer item provides space for a single line answer of up to 255 characters. This item can also validate the format of the answer:

• Alphanumeric
• Credit card expiration date
• Currency
• Numeric
• Social security number
• Text only
• URL
• Whole Number

Short Answer List

A Short Answer List item provides a variable number of single line text fields. Each text field counts as one item towards your form's limit. Text List items are limited to 100 rows each.

Short Answer Matrix

A Short Answer Matrix item provides a set of text fields in the form of a grid. Each text field counts as one item towards your form's limit.

Signature

A Signature item provides a field where form users can sign your form with their mouse or finger.

Star Matrix

A Star Matrix item allows form users to rate multiple items by selecting a number of stars. Each matrix row counts as one item towards your form's limit. Matrix items are limited to 50 rows each.

Slider

A Slider item allows form users to slide to a number in a range.

Text Field Quantity

A Text Field Quantity item must be placed directly before a Dropdown Pricing, Radio Button Pricing, Checkbox Pricing, Image List Pricing, or Text Field Pricing item. This item will assign a quantity and multiply the price of the item it's placed before.

Text Field Pricing

A Text Field Pricing item provides a text field for entering a price. Use this item if you need form users to directly enter a price.

Time

A Time item provides a text field for entering a time of day. Use this item if you need form users to directly enter a time.
