Forgot Password

First, double check you're logging in with the correct username and password.

On the login page, you can retrieve a lost username or password using the "Forgot Password" option. If a Formsite account is found that's registered to your email address, you'll be sent a "forgot password" email with further instructions. If you don't receive the email within a few minutes, double check your spam or junk email folder.

If you have multiple accounts registered to the same email address, you'll receive a separate email for each account. Check each email for the username of the specific account you're working with.
