First, double check you're logging in with the correct Formsite username and password. If you're a Sub-user, make sure to use the "Sub-user login" option to switch to the Sub-user login page.
On the login page, you can retrieve a lost username or password using the "Forgot Password" option. If an account is found that's registered to your email address, you'll be sent a "forgot password" email with further instructions. If you don't receive the email within a few minutes, double check your spam or junk email folder.
If you have multiple accounts registered to the same email address, you'll receive a separate email for each account. Check each email for the username of the specific account you're working with.

If you can't remember your account's email address or the system can't find a matching account, contact support.
If you need to gain access to an existing account that's not registered to your email address, contact support and provide us with as much information as possible, including:
- Your name and contact information
- The previous name on the account
- A link to one of your forms
- The last 4 digits and expiration date of the credit card on file
We'll work with you to establish the account's ownership.
